A client in her mid-forties tells you red wine now gives her a pounding headache, she flushes after meals she has eaten for years, and her sleep has fallen apart. It is natural to put this down to perimenopause. While this will certainly be a factor, for some clients histamine may also be part of the picture. Going undetected, this can make the situation worse for your client, and knowing how to recognise it can change how you support them.
The histamine bucket
We describe histamine tolerance as a bucket. It is filled from two sources: histamine released by the body's own mast cells (endogenous histamine) and histamine eaten in food and drink (exogenous histamine). Diamine oxidase, which is produced in the lining of the small intestine, is the main enzyme that breaks down dietary histamine before it is absorbed. When the combined histamine load outpaces the body’s ability to clear it from the system, the bucket overflows and symptoms can follow. Any factor that adds to either source, or slows clearance, lowers the histamine threshold.
How sex hormones interact with histamine
Oestradiol and histamine influence each other. In laboratory work, physiological concentrations of oestradiol prompted mast cells to release inflammatory mediators and amplified IgE-driven activation. Maintz and Novak's review of histamine intolerance describes the relationship running in both directions, with histamine able to raise oestrogen concentrations, and skin reactivity to histamine peaking around ovulation, when oestrogen is highest.
Progesterone appears to act as a counterweight. In an animal model, it inhibited histamine secretion from mast cells. In healthy premenopausal women, serum DAO was significantly higher in the luteal phase than in the follicular phase.
Why perimenopause changes the balance
Oestrogen rarely follows a steady decline in the years before the final period. Prior's review reports that oestradiol averages around 26% higher in perimenopause and fluctuates unpredictably, while ovulation becomes less reliable and progesterone insufficient or absent.
Taken together, the transition can mean more oestrogen-driven mast cell activity with less of progesterone's steadying influence. For a client whose histamine capacity was already borderline, that shift may be enough to tip the bucket over.
The symptom overlap makes it easy to miss. Histamine excess can cause flushing, headaches, palpitations, itching and digestive upset, while around 80% of perimenopausal women experience vasomotor symptoms, or hot flushes.
A note on HRT
HRT changes the hormonal picture again. A 2026 review of allergic disease in menopause notes that HRT may alter how hypersensitivity conditions develop and recommends that clinicians factor peri- and menopausal status into assessment. If a client starts or changes HRT, track whether histamine-type symptoms shift, and share what you see with her prescriber.
What to look out for in clinic
- New or worsening reactions to aged, fermented or leftover foods, or to alcohol, emerging from the early to mid forties
- Symptoms that cluster at particular points of the cycle, or became less predictable as cycles became irregular
- Flushing, headaches or palpitations that respond poorly to standard perimenopause support
- Other contributors to the bucket, including stress, alcohol (a known DAO inhibitor), certain medications and gut conditions
Blood DAO results should always be interpreted with care in any client, but especially so in premenopausal women as levels vary across the cycle.
A food and symptom diary that also records cycle day is one of the most useful tools here. A short, supervised low-histamine elimination with structured reintroduction can then help identify personal thresholds.
Where DAO supplementation fits
Where exogenous histamine is playing a role in overloading the histamine threshold, you can consider DAO enzyme supplementation.
